Your protected health information may be used or disclosed by DiabeticOne for purposes of:
For Treatment. We may use and disclose your protected health information to provide you with medical services and products. We may disclose protected health information about you to physicians, nurses or other health care entities to provide you with your supplies. For example, we may request diagnosis information from your physician to ensure that the correct supplies are being provided for treatment.
For Payment. We may use and disclose your protected health information to seek and collect payment for health care services and products we provide from your insurance provider, or from credit card

companies that you may use to pay for the services provided to you.
For Health Care Operations. We may use and disclose your protected health information, as necessary, for a variety of day-to-day activities called health care operations. For example, information on the services and products you received that may be used to support budgeting and financial reporting, to evaluate the performance of our staff, and activities to promote quality.
For Business Associates. We may use and disclose your protected health information to our business associates, such as subcontractors, so they can perform the jobs we have asked them to do. To protect your health information, we require the business associate to appropriately safeguard your health information.
Alternative Treatment/Supplies. We may use and disclose protected health information to inform you of new alternatives and products that may help you manage your health.
Marketing. We may use medical information about you to contact you by telephone or by other means to advise you of services or products that may be available. However, your protected health information will not be disclosed to third parties for marketing purposes without your prior written authorization.
Individuals involved in your care or payment for your care. We may release protected health information about you to a friend or family member who you have listed as a contact involved in your medical care. Additionally, we may give information to an individual who helps pay for your care.
For Law enforcement. We may release protected health information about you if asked to do so by law enforcement agencies to support government audits and inspections, to facilitate law-enforcement investigations, and to comply with government-mandated reporting.
As Required By Law. We will use and disclose protected health information about you when required to do so by federal, state or local law.
Lawsuits/Disputes/Court Proceedings. If you are involved in a lawsuit, dispute or court proceeding, we may disclose protected health information about you in response to a court or administrative order. We may disclose protected health information about you in response to the following: subpoenas, discovery requests or other lawful processes by others involved in the dispute. This will only be done if efforts have been made to inform you about the request or to obtain an order protecting the information requested.

Worker's Compensation. Your We may release protected health information about to comply with worker's compensation laws or similar programs that provide benefits for work-related injuries or illnesses.
Other uses and disclosures of medical information not included in this Notice or by laws that apply to its use will require your written authorization. If you provide us with permission to use or disclose medical information about you, you may revoke that authorization in writing at any time. If you revoke your authorization, we will no longer use or disclose medical information about you for the reasons covered by your written request. Note: We are unable to take back any disclosure(s) that we have already made with your authorization or pursuant to this Notice of Privacy Practices. Additionally, we are required by law to retain records of the medical services and products that we provided to you for a specific period of time.

RIGHT TO REVISE PRIVACY PRACTICES
As permitted by law, we reserve the right to amend or change our privacy practices described in this Notice. These changes in our policies and practices may be required by changes in federal and state laws and regulations. A copy of our current Notice will be posted in our offices and at our website, www.diabeticone.com. Prior to a material change in this Notice, we will promptly revise and repost it. The Notice will contain the effective date in the bottom left corner. The revised policies and practices will be applied to all protected health information we maintain.
